Form of Rocket

Form of Rocket is a math rock/hardcore band from Salt Lake City, Utah. During their existence they have gone through many lineup changes, but the core members are Peter Makowski (guitar, vocals), Tyler Smith (drums), Ben Dodds (bass, vocals) and Curtis Jensen (guitar, vocals) In 2005, Jensen left the band briefly and Gentry Densley (Iceburn, Smashy Smashy) was brought in to replace him on guitar. When Jensen returned it was to sing only. Shortly after recording their latest album MEN, Jensen left the band for good to join the Peace Corp.
