Contract Buy-out Terms and Conditions:
This Contract Buy-out Offer is limited to one Early Termination Fee credit per Sonopia subscriber account for the lifetime of the Sonopia subscriber account. Written proof of the Early Termination Fee incurred must be provided to Sonopia within 45 days of the creation of the Sonopia subscriber account. The Early Termination Fee must have been incurred subsequent to the creation of the Sonopia subscriber account.

Written proof of the Early Termination Fee must be in the form of an original or a high-quality copy of the notification or bill showing the Early Termination Fee incurred and must include all of the following:

• The carrier's name
• Your name and address, which must match the billing name and address for your Sonopia subscriber account
• Your current carrier's account number
• The specific dollar amount of the Early Termination Fee incurred
• The date the Early Termination Fee was incurred
